Standard of the “Assignment to the Healthcare Institution Providing Primary Medical Care” Public Service

1. General Provisions

1. “Assignment to the Healthcare Institution Providing Primary Medical Care” Public Service (hereinafter, the Public Service).
2. The Standard of the Public Service has been developed by the Ministry of Healthcare and Social Development of the Republic of Kazakhstan (hereinafter, the Ministry).
3. The Public Service is provided by the healthcare institutions that provide primary medical care (hereinafter, the Service Provider).
The Public Service provision applications are submitted and results are issued through:
  1) the Service Provider (at direct request or by contacting the Service Receiver);
  2) the “Electronic Government” Web-Portal www.egov.kz (hereinafter, the web-portal).

2. Procedures for the Public Service Provision

4. Term for the Public Service provision:
  1) since submission of documents by the Service Receiver to the Service Provider, as well as by making a request through the web-portal – 1 (one) business day;
  2) maximum permissible waiting time for the documents submission – 30 (thirty) minutes;
  3) maximum permissible service provision time for the Service Receiver – 30 (thirty) minutes.
  The Public Service is provided electronically on the day of request to the web-portal. At the same time, the request for the Public Service provision shall be received 2 hours before the end of the working day of the Service Provider (until 6 pmon weekdays, until 12 noon on Saturday).
5. Form of the Public Service provision: electronic (partly automated) and (or) paper-based.
6. The Public Service provision result is the Notice (Counterfoil) of Assignment in paper form (free form) or in the form of e-document signed with electronic digital signature (hereinafter, EDS) of the Service Provider as per the Annex to this Standard of the Public Service.
7. The Public Service is provided free of charge.
8. Operating Schedule:
  1) the Service Provider – Monday to Saturday (Monday – Friday from 8 amto 8 pm without any breaks, on Saturday from 9 am to 2 pm, except for weekend (Sunday) and holidays according to the Labor Code of the Republic of Kazakhstan;
  2) the Web-Portal – 24 hours, except for technical timeouts associated with maintenance works (at the request of the Service Receiver after the end of the working hours, on weekends and holidays according to the Labor Code of the Republic of Kazakhstan, the Public Service provision applications are submitted and results are issued on the next business day).
9. List of documents required for the Public Service provision at the request of the Service Receiver (or its representative based on the Power of Attorney):
  1) for the Service Provider:
  free form application;
   identity document to identify the entity in the direct request;
  2) for the Web-Portal:
  request in the form of an e-document certified by the EDS of the Service Receiver.
The Service Provider receives information on the identity documents from the relevant public information systems thought the gateway of the “e-government”.

3. The Procedure for Appealing against the Decisions, Actions (Inaction) of the Service Provider, and (or) Its Officials on the Public Service Provision

 10. The decisions, actions (inaction) of the Service Provider, and (or) its officials on the Public Service provision shall be appealed by filing a complaint to the Head of the Service Provider or the Ministry at the address indicated in paragraph 12 of this Standard of the Public Service, or at the following address: 010000, Astana, 8 Orynbor Street, House of Ministries, Entrance No. 5.

 Confirmation of the complaint submission is its registration (stamp, reference number and date) in the Office of the Service Provider or the Ministry indicating the last name and initials of the person who received the complaint, the period and place for the reply to the complaint. After registration, the complaint is forwarded to the Head of the Service Provider or the Ministry to determine the responsible officer and take appropriate action.

 The Service Receiver’s complaint received at the address of the Service Provider or the Ministry is subject to review within five working days from the date of its registration. Reasonable reply on the outcome of the complaint review is sent by mail or issued to the Service Receiver in the office of the Service Provider or the Ministry.

 When requesting via the web-portal, information on the appeal procedures is available for the Service Receiver through the single contact center for the issues of the Public Service provision by calling 8-800-080-7777, 1414.

 When sending an electronic request through the web-portal, information on the request, which is updated during processing of the request by the Service Provider (delivery, registration, execution status, the reply to the complaint), is available for the Service Receiver from the "Personal Account".

 In case of disagreement with the results of the provided Public Service, the Service Receiver files a complaint to the authorized body for assessing and monitoring the quality of public services provision.

 The complaint of the Service Receiver received at the address of the authorized body for assessment and quality control for public services provision is reviewed within fifteen working days from the date of its registration.

 11. In cases of disagreement with the results of the provided public service, the Service Receiver addressed to the court in the manner prescribed by the laws of the Republic of Kazakhstan.

4. Other Requirements Given the Specifics of the Public Service Provision, Including Provided Electronically

 12. The addresses of the places of the public service provision are available on the website of the Ministry www.mzsr.gov.kz, “Public Services” section.

 13. The Service Receiver can obtain the public services electronically via the web-portal subject to availability of EDS.

 14. The Service Provider can remotely obtain information on the procedure and the status of the Public Service provision through "Personal Account" of the web-portal, as well as through the single contact center on the issues of the public service provision.

 15. Contacts of inquiry services on the Public Service provision are listed on the website of the Ministry: www.mzsr.gov.kz. The single contact center on the Public Service provision: 8-800-080-7777, 1414.
