According to the official website of capital akimat, goal of program implementation is the management of the main chronic diseases patients on an outpatient basis. This project will improve the quality, life expectancy of patients, teach to manage their own health.

Since 2017, the program has been operating in 15 polyclinics of the city. The capital has started to implement the Address of the Head of state, in which the priority direction is the quality of medical services.

Created a multidisciplinary team in the pilot clinics, the roles in the team, responsible for keeping the register, segmentation of patients, scheduled call, scheduled appointment, filling out a card observation of patients, preparation of a brief plan of action.

By the end of the year, the department plans to cover patients involved in the disease management program up to 20% of the total number of dispensary patients (hypertension, diabetes mellitus (type 2), chronic heart failure) and reach 30% by the end of 2019.
